This is a rare thing in Japan, a 100% vegan restaurant that also offers baked goods to go.
The menu is a eclectic selection of Japanese and other Asian cuisines, including such things as (Japanese) curry and tempeh spring rolls. They also offer a variety of vegan baked goods and desserts, including muffins and tofu cheesecake.
The menu is in both English and Japanese.
Take the Hibiya subway line to the Hiro-o station. Then go out either exit 1 and take a right, or exit 3 and take a right. It is located between those two exits.

Wow, a vegan restaurant in Tokyo. I've only been here once but I enjoyed the food quite a bit. I had the curry, which was Japanese curry with wheat gluten, potato, carrot, and lotus root. It was quite tasty, as was the salad it came with.
I also got the almond tofu cheesecake for dessert. It wasn't very almond-y, but it had a nice lemony flavor and a good texture. I've had better tofu cheesecake, but this was quite decent, and an incredibly rare thing to find in Tokyo.
